Azure IoTHub integration is broken

Hello team,i have followed the Integration Guidemultiple times, deployed the template and everything, after it’s all done my devices show up but are always disconnected, and i see no telemetry also the TTN console shows the error below:

i’ve search google as well and couldn’t find any guides or tutorial to do this…

can anyone please help…
